How to Report a Claim

Through the Monoline Platform (Recommended)
  1. Log in to your account at app.monoline.com
  2. Search for the customer's policy
  3. Open the policy page
  4. In the Claims section, select Start a Claim
  5. Complete the First Notice of Loss (FNOL) submission

Please include:

  • Monoline Policy Number
  • Named Insured
  • Date of Loss
  • Type of Loss
  • Details of Claim
  • Underlying carrier claims adjuster contact information (phone & email)
  • Attach a full copy of the underlying policy
